Reject Imperfection is an instant that targets a spell. When it resolves, it first attempts to counter that spell. If the countered spell had mana value 3 or less, the spell's controller (the controller of Reject Imperfection) then proliferates. Proliferate: choose any number of permanents and/or players that already have counters on them (you can choose zero). For each chosen object or player, put one additional counter of each kind already present on that object or player. You can choose any applicable permanents or players, including those of opponents. You cannot choose cards not on the battlefield. If a permanent ends up with both +1/+1 and -1/-1 counters, the game immediately removes them in pairs as a state-based action so only one kind remains if possible.

To cast Reject Imperfection you target a spell on the stack. It is an instant, so you can cast it any time you could cast an instant.
When Reject Imperfection resolves, first it attempts to counter the targeted spell. If that spell is countered, it goes to the appropriate zone (usually its owner's graveyard) and it is no longer on the stack.
If the countered spell's mana value was 3 or less, you then proliferate. Proliferate means: choose any number of permanents and/or players that already have at least one counter on them (you may choose zero). For each chosen permanent or player, give it one additional counter of each kind of counter it already has.
You may choose permanents or players controlled by any player, including opponents. You may not choose cards not on the battlefield even if they have counters (for example, counters on cards in graveyards or on emblems on the stack can't be chosen).
If a permanent has multiple kinds of counters (for example, a +1/+1 counter and a -1/-1 counter), proliferating gives one of each kind it already has; rules then remove +1/+1 and -1/-1 counters in pairs as a state-based action if they exist simultaneously.

Częste pomyłki

Thinking proliferate adds only one type of counter when a permanent has multiple kinds: proliferate adds one of every kind already present on each chosen object or player.
Believing you must choose every permanent or player with a counter: you may choose any number, including zero; you choose only the ones you want.
Trying to choose counters on cards not on the battlefield (like counters on cards in graveyards): you can only choose permanents on the battlefield and players.
Assuming you can respond after the proliferate choices are made: once the spell starts to resolve and the choices for proliferate are made, it's too late to respond to that proliferate effect.

Example: You cast Reject Imperfection targeting an opponent's spell with mana value 2. It resolves and counters that spell. Because its mana value was 3 or less, you proliferate. You choose three permanents: one with a +1/+1 counter, one with a loyalty counter, and an opponent who has a poison counter. The +1/+1 permanent gets one more +1/+1 counter, the planeswalker gets one more loyalty counter, and the opponent gets one more poison counter.

If a permanent ever has both +1/+1 counters and -1/-1 counters on it at the same time, they're removed in pairs as a state-based action so that the permanent has only one of those kinds of counters on it.

Players can respond to a spell or ability whose effect includes proliferating. Once that spell or ability starts to resolve, however, and its controller chooses which permanents and players will get new counters, it's too late for anyone to respond.
