Then create a 0/1 white Human creature token with the static ability 'Permanents can't phase in.' Important: 'Phase out' makes those permanents be treated as though they don't exist while they are phased out. Any Auras or Equipment attached to phased-out permanents become unattached and remain on the battlefield. The token's ability prevents phasing-in events while the token is on the battlefield; it does not retroactively affect the already completed phasing-out. II — When this chapter resolves you draw two cards, then each player loses 2 life (these happen in that order). III — When this chapter resolves, you may sacrifice a creature token as part of the effect. If you do sacrifice a creature token, you gain 5 life. Whether or not you sacrificed a creature token, the Saga is sacrificed after the III ability finishes resolving.

Saga triggers: When The City on the Edge of Forever enters the battlefield, it gets a lore counter and its I ability triggers. After your draw step on subsequent turns, it gets another lore counter and triggers the next chapter ability. When it gets the third lore counter, you sacrafice it after resolving III (see 'Sacrifice after III' below).
Phasing (I): When I resolves, all artifacts and creatures phase out. Phased-out permanents are treated as though they don't exist — they can't be targeted, don't affect the game, and attachments (Auras, Equipment) that are attached to them become unattached and follow normal rules for attachments. The token created by I has "Permanents can't phase in." so phased-out permanents can't phase in during the phase they would otherwise phase in while that effect applies.
Token's replacement effect: The 0/1 white Human creature token created by I has the static ability "Permanents can't phase in." That ability prevents permanents from phasing in while the token is on the battlefield. It does not itself cause phasing; it only stops phasing-in events.
Phased-out permanents and attachments: Treat phased-out permanents and anything attached to them as though they don't exist. If something phased out had an Aura attached, that Aura becomes unattached when its object phases out; it remains on the battlefield (if legal) and could move or be attached later if allowed.
Timing of phasing and the token: The token is created by the I ability after all artifacts and creatures phase out. The token exists on the battlefield during the same resolution, so its static ability can affect future phasing-in events, but it does not retroactively stop the phasing-out that already happened.
II resolution: When II resolves, you draw two cards then each player loses 2 life. These are sequential effects on the same ability (draw cards, then players lose life).
III resolution and Sacrifice after III: The text says 'Sacrifice after III.' That means the Saga is sacrificed after III's ability resolves. III's ability reads: 'Sacrifice a creature token. If you do, you gain 5 life.' You choose whether to sacrifice a creature token as part of the resolution. If you sacrifice a creature token as part of III's effect, you gain 5 life. Regardless of whether you sacrificed a creature token, after the III ability finishes resolving the Saga is sacrificed and goes to the graveyard.
If you can't sacrifice a creature token for III: If no creature token exists when III resolves, you simply can't perform the 'Sacrifice a creature token' part, so you don't gain 5 life. The Saga is still sacrificed after III resolves.
Interaction with replacement and replacement-like effects: The Saga's chapter abilities are triggered and resolve normally. If a replacement or prevention effect would change a part of those abilities, apply them per normal rules. For example, if an effect prevents life loss, it applies to the II step where each player would lose 2 life.
Continuous nature of 'Permanents can't phase in.': That is a continuous static effect provided by the token while it's on the battlefield. If the token later leaves the battlefield, that continuous effect ends and phased-out permanents that would normally phase in on the next appropriate event can do so (subject to other effects).
