Kagemaro's power and toughness continuously update to match the current number of cards in your hand. If you draw or discard, its stats change immediately.
To activate the sacrifice ability, pay and sacrifice Kagemaro as part of paying costs. The ability goes on the stack. When the ability resolves, determine X by counting the number of cards in your hand at that moment, then give all creatures -X/-X until end of turn.
Because Kagemaro is sacrificed as a cost, it is not on the battlefield when the ability resolves. Its power/toughness at resolution does not affect X — only the number of cards in your hand at resolution matters.
If X is 0 (you have no cards in hand when the ability resolves), the effect gives all creatures -0/-0, which has no effect.
The -X/-X effect applies to all creatures, including your own creatures (and any copies or token creatures). It also affects creatures that enter the battlefield after the ability resolves, for the remainder of the turn.
Replacement and continuous effects that modify power/toughness apply normally; the -X/-X is a layer effect that modifies power and toughness and interacts with other effects in the usual way.
